Palliative Care Community Team (PCCT)
The Palliative Care Community Team (PCCT) works with various community partners and healthcare organizations to support individuals and their families within a palliative approach to care, including emotional, psychosocial, and spiritual support. We are available to support people diagnosed with any progressive life-limiting illness from time of initial diagnosis through bereavement.

Hospice Navigator
Our Hospice Navigators are Registered Nurses with expertise in hospice palliative care; They are members of the healthcare team who assist individuals and families in navigating the healthcare system throughout the illness journey.
Hospice Navigators can provide psychosocial support, education on disease trajectory and symptom management, assist with Advance Care Planning, offer resources, and advocate on the behalf of individuals living with life-limiting illness and their caregivers. Hospice Navigators assess and support clients using a holistic model based on physical, psychological, social, spiritual, emotional and practical needs. The Hospice Navigator may guide individuals and families to appropriate community supports and resources, as well as other services within VON Durham Hospice Services.
Supportive Care Counsellor
The Supportive Care Counsellor is a skilled registered professional who therapeutically provides support and helps assist individuals, family members and caregivers to understand and cope with the emotional, psychological, social, spiritual and practical challenges that may arise when experiencing a life-limiting illness, or a death.
The Supportive Care Counsellor is available to provide advocacy, grief and bereavement supports, information and resources, counselling and referral services. Supportive Care Counselling assists individuals and their families to find meaning with their situation using a holistic approach.
